I, Dennis Zdenek Kriz, declare under penalty of perjury that I understand written (Czech, French, Italian, Brazilian Portuguese, Spanish); that I am capable of writing competently in (Czech, French, Italian, Brazilian Portuguese, and Spanish) as well as in the English language; that I have years of experience in providing accurate written translations between these languages; and that, to the best of my knowledge and belief, the statements in the attached translation of _____________, consisting of _____ pages (which I have initialed ______), have the same meanings as the statements in the language in the original document, a copy of which I have examined.
